This Privacy Statement describes and applies to the information you provide, or we collect from you when visiting or using voice, mobile, or desktop devices to access Banner Health platforms, tools, and applications, BannerHealth.com, and other Banner Health websites (collectively, “Services”). This Statement also explains how we use the information, when we disclose it, and how you can manage the information. This Privacy Statement is part of Banner Health’s Terms of Use.
This Privacy Statement does not apply to protected health information (PHI) that Banner Health obtains in connection with the provision of health care or other health care-related services to patients. PHI is subject to the Health Insurance Portability and Accountability Act (HIPAA) and Banner Health’s Notice of Privacy Practices.
When you use the Services, we receive and collect certain information that may be linked to or reasonably linkable to you, for example, your name, email address, billing address, shipping address, phone number, social security number, and credit card information (“Personal Data”). The Personal Data that we receive and collect depends on what you do when you use the Services. The Personal Data includes automatically collected information and information you actively submit as described below.

In order to use some of the Services, you will need to provide Personal Data. For example, Personal Data is required when you want to do things like (i) submit a job application; (ii) make an online donation; (iii) sign up for a class, event, or other service; (iv) contact us; (v) participate in a text or chat session; (vi) purchase merchandise from the Banner Store; (vii) reserve a spot or make an appointment to receive services offered by Banner Health; (viii) pre-register for services; (ix) create an account; or (x) access other digital services.
Personal Information about Children.
In limited circumstances, some Services can be utilized by children who have not reached the age of majority, which is 18 years of age in most states. This includes, but is not limited to, scheduling appointments relating to Services that the child can consent to receive and providing health information. Other than as noted above, if you have not reached the age of majority, do not use, or provide any information to obtain Services. If you believe we may have improperly received information from or about a child who has not reached the age of majority, please contact us. The Services are not intended for and may not be used by children under the age of 13. If we learn that a child under the age of 13 has provided Personal Data to us, in accordance with the Children's Online Privacy Protection Act (see the Federal Trade Commission's website for more information about this Act), we will delete the child's Personal Data from our files to the extent technologically possible and permissible under applicable laws.

We use reasonable and appropriate physical, technical, and administrative security measures and safeguards to protect the confidentiality and security of Personal Data. We ensure that sensitive Personal Data is encrypted where possible or required by law. Access to Personal Data is limited to individuals who need access and appropriate security is in place to avoid unauthorized sharing of Personal Data.
While we take reasonable security measures to protect Personal Data, we cannot guarantee its security. We cannot promise or guarantee that hackers, cybercriminals, or other unauthorized third parties will not be able to defeat our security, and improperly collect, access, steal, or modify Personal Data.

When, through utilizing the Services, you click on links that take you to a third-party application or website, you will be subject to the applicable third-party’s terms of use and privacy policy. Banner Health accepts no responsibility for any website, application, or other tool owned and operated by a third party (collectively, “third-party offerings”) or for any loss or damage that may arise from your use of third-party offerings . If you decide to access a third-party offering, you do so entirely at your own risk and subject to the terms and conditions of use for the third-party offering. We encourage you to read the posted privacy statement of any and every site you visit, whether you are linking from our Services or browsing on your own.
We believe it is important for you to be able to find out what Personal Data we have collected through the Services and to manage that Personal Data. The Services are intended for use only by residents of the United States. If we change this Privacy Statement, we will post the most recent version of the Privacy Statement on BannerHealth.com so you are always aware of the information we collect, how we use it, how we protect it, and when we disclose it.
